Which Patients Qualify?

To qualify for reimbursement for patient self testing, your patient must:

  • Have a mechanical heart valve, chronic atrial fibrillation, primary hypercoaguable state or venous thromboembolism — including deep vein thrombosis (DVT) and pulmonary embolism(PE)
  • Be taking Coumadin® (warfarin) for a minimum of three months

But not every patient who qualifies will be a good candidate for self testing. The ideal candidate for patient self testing is:

  • Motivated and interested in self testing
  • Patient or caregiver physically capable of performing a self test (after training)
  • Restricted from making frequent lab visits due to:
    • Distance
    • Caregiver limitations
    • Work or personal schedules
    • Frequent travel
    • Part-time residency in the area ("snow birds")
  • Unable or prefers not to provide a venous blood sample
